Random Drug/Alcohol Testing Agreement

I understand that Assurance Home & Convalescent Care, Inc. is a drug and alcohol free workplace with zero tolerance for abuse of legal or illegal drugs and/or alcohol.

Furthermore, I understand that during the course of my employment with Assurance Home & Convalescent Care, Inc. I may be required to undergo random drug and/or alcohol testing as a condition of continued employment with the company.

By signing below, I agree to undergo random drug and/or alcohol testing at the request of a Director of Assurance Home & Convalescent Care, Inc.

Administration of Medication Policy & Caregiver Understanding Non-Compete Agreement 

I understand that as an employee of Assurance Home & Convalescent Care, Inc., I am prohibited from administering medication, prescribed or over-the-counter, to any client of Assurance Home & Convalescent Care, Inc.  I agree to abide by this policy.

I understand that as an employee of Assurance Home & Convalescent Care, Inc. I am prohibited from encouraging, enticing or coercing any client from the services of Assurance Home & Convalescent Care, Inc.  I agree not to accept work independently from an established Assurance Home & Convalescent Care Inc. client.  In addition, I agree not to encourage, entice or coerce an established Assurance Home & Convalescent Care, Inc. client to accept independent services from any of my friends or relatives.
